Responsibilities

• Perform quality routine nursing care to assigned patient; may include patients with G-Tubes, Trachs, Vents and CVLs.

• Accompany patient to school to provide care during the school day; may also include home hours before and after school. 

• Monitor and act on patient progress and identify any changes in status.

• Perform patient teaching/training for patients and families.

• Communicate with all staff, patients and families with excellent written and verbal skills.

• Comply with all policies and procedures, while upholding the expectations of Horizon Care Partners

Qualifications

• Valid LPN/RN nursing license in Georgia or Nurse Licensure Compact (NCL)

• Active CPR certification (ONLINE COMPANIES ARE NOT ACCEPTABLE)

• Two or more years of experience is preferred; new graduates accepted based on completion of preceptorship

• Valid PPD skin test
